Increased Access to Meals for Seniors

This collaboration strengthens the frontline response to a crisis affecting millions of aging adults across the country and thousands in Yolo County alone.
“In a state where more than 735,000 seniors face food insecurity, and more than 8,000 in Yolo County, Meals on Wheels Yolo County is creating healthier outcomes for Yolo County seniors through the delivery of nutritious food which also fosters social connections through regular visits and interactions between volunteers and seniors,” said Susan Rawlings Molina, co-Founder and CEO at GroundGame.Health. “We’re honored to power their work with technology that helps them advance their ability to drive deeper local impact.”
Meals on Wheels Yolo County serves more than 1,000 seniors daily, bridging nutritional and social gaps across both rural and urban communities.
Meals on Wheels Yolo County is also expanding its reach through other nutritional assistance programs such as medically tailored meals to Medi-Cal members with chronic conditions as well as meal provision relationships with other regional senior nutrition programs and community-based organizations to help even more individuals in Northern California.

Yolo County holds one of the highest poverty rates in California at 19.5%, despite being one of the nation’s richest agricultural regions. Many seniors live in food deserts or face mobility challenges that isolate them from essential services. Meals on Wheels Yolo County addresses these challenges head-on—now with even greater capacity through GroundGame.Health’s Implify platform.
Through this partnership, Meals on Wheels Yolo County will use Implify™ for the day-to-day management of the individuals they serve, the services they provide and the volunteers who support them.
This includes staff/volunteer management and scheduling, service planning and fulfillment, outreach, engagement, education, navigation and streamlined revenue-cycle management services. Critically, the platform supports services backed by both the Older Americans Act and California Advancing and Innovating Medi-Cal (CalAIM), aligning service delivery with regulatory and funding frameworks while reducing administrative burden.
